3. 02 Figure 8 1e Winch Installation Step 03 Page 22 500995 001 Product Manual LG5 Series Manhole Guard Retrieval System Revision 04 Date 03 04 2009 8 2 Kick Plates PVC Strip This Kick Plate accessory inclusive of PVC Striping and standard fasteners can be fitted to the LG5 Series system This accessory provides protection against debris or objects that can fall into the manway The kick plates can be fitted on all three bottom cross bars of the lower section of the manhole guard as shown in the figure below The kick plates can be ordered from Pelsue with the Part Number 500908 001 a gt Kick Plates Figure 8 2 LG5 Series System with Pelsue Kick Plates Page 23 500995 001 Product Manual LG5 Series Manhole Guard Retrieval System Revision 04 Date 03 04 2009 8 3 Replacement Rubber Feet The replacement rubber foot accessory inclusive of rubber feet with recessed washers and standard fasteners can be fitted to the LG5 Series system This accessory can replace the leveling feet that come installed on the system if desired The rubber feet can be used to prevent slippage between the system and the ground To use the replacement rubber feet the leveling feet must first be removed by un threading it all the way until the feet are detached from the guard The rubber feet can now be threaded in to take the place of the original feet as shown in the figures below The rubber feet must be threaded
4. 04 Date 03 04 2009 Page 17 6 2 Inspection continued SS SE aa A 3 Overhead Anchor Points Ensure that the two overhead anchor points swivel freely and that there is no binding in their range of motion Also visually inspect the anchor lugs for any sign of deformation The anchor lugs can elongate and stretch if overloaded beyond the capacity of the system Any Model LG5 system exhibiting deformation in the anchor lugs should be removed from service immediately 4 Structure The entire product structure should be checked for cracks dents bends or breaks If there are major dents or any other structural damage the unit should be removed from service and returned to the factory for repair Pay special attention to weld locations look for cracks or separation at the welds Keep in mind the powder coating can age and crack this normal paint cracking can be confused with structural cracking If there is any uncertainty with regards to cracks contact the Pelsue service department for assistance and or recommenda tions Ensure that the upper and lower assemblies can be separated and folded as intended Inability or difficulty in folding the structure as intended may be indicative of structural damage f the unit fails to fold or stow as intended remove the product from service and investigate further Likewise the rear locking bar should fold up and down easily binding of the locking bar may also indicate a structural deficienc

T A Pelsue Parts and Ser vice Department in Englewood Colorado or at the option of the T A Pelsue Company service may be performed at any designated service center which may include any authorized service center for the component manufacturer Unauthorized repairs will not be covered by this war ranty Damage or failure due to misuse mishandling or unauthorized modifications will not be covered by this warranty Unless otherwise agreed the T A Pelsue Company shall repair or replace the defective components within 30 calendar days of notice of failure T A Pelsue Company s obligation hereunder shall be limited to the repair or replacement of the product or component as set forth above and shall not include any liability whatsoever for damages caused by such failure including but not limited to consequential or incidental damages flowing from use or lack of use of product Any replacement repair modification installation or other service performed by the T A Pelsue Company shall be warranted by the remainder of the unexpired period of the warranty or for a period of 90 calendar days whichever is longer All materials or parts returned for credit or warranty shall be returned only with prior approval and will be subject to factory inspection before credit is allowed Parts claimed defective will be replaced upon request and will be invoiced as purchased subject to credit when the parts claimed to be defective have been receiv

14. to increase the frequency Factory servicing shall include but not be limited to an intensive inspection and cleaning of all internal and external components Failure to provide proper service may shorten product life and could endanger performance or function 6 2 Inspection When completing an inspection of the Model LG5 Manhole Guard Retrieval System the following specific items should be checked 1 Labels Check that all the labels are clean and legible Refer to Section 3 1 Clean the labels if any are dirty using mild soap and a damp cloth Replace if any are illegible Decals labels provide valuable warning and caution infor mation applicable to product usage and limitations and should be visible and present at all times 2 Fasteners Check that all screws and other fasteners are present and secure Note that not all fasteners are tightened to their maximum allowable torque This is done in order to allow for pivoting and folding motions in certain components Over tightening of fasteners can interfere with the operation of the device and can damage the structure All fasteners on the Model LG5 system are of a self locking design Bolt threads should extend all the way though their locking nuts If any fasteners have appeared to back out or loosen the product should be removed from service until the problem can be identified and corrected 500995 001 Product Manual LG5 Series Manhole Guard Retrieval System Revision

ANSI and OSHA require a regular inspection program during the inspection Return to an authorized repair depot for all confined space entry retrieval equipment and to or the factory for service maintain documented results of these inspections Follow the inspection procedure contained in this manual and use the inspection form to document the results 6 1 Maintenance For the most part the Model FLG5 is intended to be a maintenance free piece of equipment however several steps can be taken to keep the system in top notch working order and also extend the working life of the product Before Each Use 1 Visual Inspection Perform a complete visual inspection Refer to section 6 2 for guidelines and take any maintenance steps that are necessary Annually 1 Clean Product Use a damp cloth and mild soap to clean the structure and labels of dirt and residue Be sure the labels are legible replacements are available from Pelsue 2 Complete Inspection Perform a thorough inspection paying attention to the bullet points provided within section 6 2 Record results and keep documentation A log has been provided in section 6 3 specifically for the recording of the dates 8 results of annual system inspections Periodic Factory Service Inspection t is recommended that the product be serviced by a factory authorized service center or the manufacturer after a period of three years Extreme working conditions may indicate the necessity
