1. anchorage point which is far to the left or right of the user in order to minimize swing fall type injuries 2 The Skyhook Swivel Top fastens to the top of the Skyhook Anchor without attached horizontal lifeline when the hole in the Skyhook Anchor is aligned with the Skyhook s top attachment 3 When the Skyhook Swivel Top s lower attachment hole is aligned with the Skyhook Anchor s larger top attachment insert the locking hitch pin and secure the pin s locking pin 4 Ensure that the Skyhook Swivel Top is properly installed by attempting to pull the Skyhook Swivel Top off of the Skyhook Anchor INSTALLATING THE RETRACTABLE LIFELINE The Guardian Skyhook Swivel Top is designed to accept numerous manufacturer s retractable lifelines 65 foot length maximum 1 Removed clevis pin from retractable receiver wings 2 Slide fall block horizontally into position 3 Reinsert clevis pin and insert hitch pin 4 Inspect and operate retractable according to manufacturer s instructions Guardian Fall Protection Kent WA 800 466 6385 www guardianfall com Page 9 GUARDIAN FALL PROTECTION INSTALLATING THE RETRACTABLE SWIVEL ADAPTOR When using 50ft and 65ft retractable lifelines the retractable swivel adapter is required Guardian Part 10975 1 Before installing the Skyhook Swivel Top attach the three sections of the adapter as shown below 2 Secure them with the included bolts 3 Then proceed with installation of the S2. L PROTECTION LIMITATIONS CAPACITY The Skyhook Swivel Top is designed for use by persons with a combined weight clothing tools etc or no more than 310 Ibs but up to 400 Ibs using an approved connecting system No more than one personal protective system may be connected at one time FREE FALL Personal Fall Arrest Systems PFAS used with this equipment must be rigged to limit the freefall to six 6 feet as called out in ANSI Z359 1 Only qualified and personnel trained on the proper use of fall protection such as this anchor are allowed to use this product Restraint systems must be rigged so that no vertical free fall is possible FALL CLEARANCE There must be sufficient clearance below the use to arrest a fall before the user strikes the ground or any other obstruction At least 20 of clearance is needed for fall arrest systems A hazard assessment by a trained and Competent Person is recommended before any work is started that would include the use of fall protection SWING FALLS Swing falls occur when the anchorage point is not directly above the point where a fall occurs The force of striking an object in a swing fall may cause serious injury or death Minimize the risk of swing falls by working as close to the anchorage point as possible Do NOT permit a swing fall if injury could occur Swing falls will significantly increase the clearance required when a self retracting lifeline or other variable length connecting system is us
5. ained in self rescue or alternate means shall be provided for prompt rescue in the event of a fall ANY EQUIPMENT SUBJECTED TO THE FORCES OF A FALL ARREST MUST BE REMOVED FROM SERVICE IMMEDIATELY CONTACT YOUR DISTRIBUTOR OR GUARDIAN ABOUT POLICIES REGARDING REPLACEMENT OF GUARDIAN COMPONONENTS INVOLVED IN A FALL DESCRIPTION OF GUARDIAN SKYHOOK SWIVEL TOP The Guardian Skyhook Swivel Top is a roof mounted fall protection system designed for use on Skyhook Rooftop Anchors It allows workers free movement while tied off to an approved and already installed Skyhook Anchor Guardian Fall Protection Kent WA 800 466 6385 www guardianfall com Page 4 GUARDIAN FALL PROTECTION MAINTENANCE CLEANING AND STORAGE e Repairs to the Guardian Skyhook Swivel Top can be made only by a Guardian representative or person or entity authorized by Guardian Contact Guardian for maintenance and repair e Cleaning after use is important for maintaining the safety and life of the equipment e Cleanse the equipment of all dirt corrosives and contaminants e If the Guardian Skyhook Swivel Top cannot simply be wiped clean use a mild soap and water rinse wipe to dry e Store the Guardian Skyhook Swivel Top where it cannot be affected by heat light excessive moisture oil chemicals or other degrading elements INSPECTION e Before each use the worker must inspect all fall protection equipment e A formal inspection must be made by a Com
6. cking snaphooks and carabiners with any Guardian equipment Do not use equipment that is not compatible If you have any questions on compatibility please call Guardian LARGE THROAT OPENING SNAPHOOKS SHOULD NOT BE CONNECTED TO STANDARD SIZE D RINGS OR SIMILAR OBJECTS WHICH WILL RESULT IN A LOAD ON THE GATE IF THE HOOK OR D RING TWISTS OR ROTATES LARGE THROAT SNAPHOOKS ARE DESIGNED FOR USE ON FIXED STRUCTURAL ELEMENTS SUCH AS REBAR OR CROSS MEMEBERS THAT ARE NOT SHAPED IN A WAY THAT CAN CAPTURE THE GATE OF THE HOOK ANCHORAGE STRENGTH REQUIREMENT The anchorage strength required is dependent on the application Following are anchorage strength requirements for specific applications Ensure that any anchorage point used in the personal fall protection system meet the following requirements Please note that these requirements are for single anchor points and not HLL Systems e FALL ARREST The structure to which the anchorage connector is attached must sustain static loads applied in the directions permitted by the fall arrest system of at least 3 600 Ibs with the certification of a Qualified Person or 5 000 Ibs without certification Refer to OSHA and ANSI for specific definition This anchor is to be used by one worker only Do not tie off equipment Anchorages used for attachment of a personal fall arrest system shall be independent of any anchorage being used to support or suspend platforms e RESTRAINT The structure to which the ancho
