Bulletin Number FSN013 Urgent Field Safety Notice Invacare SB750 Date 31 August 2008 Ref 2007 01 1 015 061 004 Urgent Safety Information In the interests of the safety of our customers we would like to bring your attention to a single known incident where a patient became entrapped between the foot section and the bed end The user was of particularly restless nature and the knee break section had been left in the raised position This incident would have been prevented if the bed had been placed in the lowest position as advised in the user manual To improve safety while using the SB750 and to make users aware of correct use of the product it is recommended that the user attendants follows precisely the instructions for use as described in the product user manual i e Warnings on page 6 and 8 of user manual The user manual has been amended to emphasise these warnings The amended user manual is available via Invacare Website Additionally for users of a restless disposition unattended patients etc further measures have been taken and product modifications have been developed A limitation crossbar is now available part number 1510820 1015 that can be fitted to the bed which prevents the leg section mattress support falling under horizontal level Please note that there will be a charge for this upgrade Please note that this is considered an isolated incident and the risk of entrapment negligible when the SB7502. is used in accordance with the instructions provided in the user manual The benefit to the patient of the knee break feature of the bed considerably outweighs the residual risk Please ensure that the information in this notice is made available to all relevant personnel within your organisation and or customer base and that awareness is maintained for an appropriate period If you have any questions relating to this bulletin or should you require any additional information please contact Technical Service Department at Invacare Technical Services Department Tel 01656 647327 Fax 01656 753299
