17. medication should be selected for all drugs typically prescribed for heart disease except for blood pressure medication Check the class for which the drug is most typically used In some instances this may not be the reason for which the drug is prescribed For example Codeine should be checked as pain killer opiate even if it was prescribed as a cough suppressant Street drugs include recreational drugs not generally available by prescription such as heroin cocaine LSD and methamphetamine If the child died from a prescription drug that was obtained illicitly please list the agent in the appropriate prescription drug category In order to obtain data on the exact agents involved in poisoning deaths among children and youth please indicate the actual trade or generic name of each agent contributing to the death in the other specify field for the corresponding category as well as in the narrative For example if codeine was one of the contributors to the death check pain killer opiate and write codeine in the other specify field G9b Where was the substance stored Specify place substance was stored at time of incident This may not be applicable in all cases such as recreation drug use In these situations leave the item blank G9c Was the product in its original container Code as yes if poison was in the container that the manufacturer originally packaged it in or in its original prescription dispensing format G9d

Review Case Reporting System Depending on your permission level set by your State Administrator you may or may not be able to generate local level reports or state level reports How to Generate a Report The main Reports Page contains the selection criteria for the reports and the menu list of the reports that you can run grouped into similar types of information Selection Criteria The standardized reports are generated by the year of case review or by year of death The standardized reports also have the ability to filter cases based upon whether the cases have been marked complete for data entry This is the check box Data Entry Completed For This Case in Section N of the report If you would like to run a report for all cases reviewed between 2004 and 2007 select 2004 for the start review year in the drop down box and 2007 for the end review year in the drop down box see first example below If you would like to run a report that included all cases where the year of death was 2006 select 2006 for the start death year and 2006 for end death year see second example below You must select either All Cases or Cases marked complete for data entry Likewise you must select either Year of Review or Year of Death in the selection criteria If you select Year of Death as your selection criteria the reports will automatically exclude those cases that do not have a year of death entered into the data tool A

202. w or by year of death The standardized reports also have the ability to filter cases based upon whether the cases have been marked complete for data entry This is the check box Data Entry Completed For This Case in Section N of the report If you would like to run a report for all cases reviewed between 2004 and 2007 select 2004 for the start review year in the drop down box and 2007 for the end review year in the drop down box see first example below If you would like to run a report that included all cases where the year of death was 2006 select 2006 for the start death year and 2006 for end death year see second example below You must select either All Cases or Cases marked complete for data entry Likewise you must select either Year of Review or Year of Death in the selection criteria If you select Year of Death as your selection criteria the reports will automatically exclude those cases that do not have a year of death entered into the data tool A statement will appear that advises you of the number of cases with missing year of death information You can select the case type that you would like included in your reports This is the question found on the Case Definition data entry page with the options of child death child near death event or not born alive The selection criterion also has the option to view all cases Selection Criteria All cases Cases marked
