10. pass containing details of the pacemaker model function frequency electrode type etc 2 AS a general rule the electro medical devices used in dental practices can cause interference a Brush type electric motors generate magnetic and electric fields which can influence cardiac pacemakers under certain circumstances The proximity of the motor to the pacemaker plays an important role in this respect Conventional drive units for drills etc do not exert any influence b Electro surgery procedures can also influence cardiac pacemakers These influences depend on the frequency spectrum and on the extent to which the pacemaker electrodes are located within the interference tield within the treatment site In extreme cases ventricular fibrillation can be provoked coupled with the permanent failure of the pacemaker For this reason the patient should be continuously monitored and a defibrillator should be kept available for immediate use Electro surgery procedures can cause interference within a radius of two metres intervening walls do not provide any protection This can lead to an irregular pulse and even to ventricular fibrillation In such cases pacemaker patients should be admitted to hospital before undergoingelectro surgery c Dental vitality tests with the aid of electrical devices present no problems To the extent that interference occurs it is short lived and of only minimal intensity d According to the scientific liter

13. that immediate electroshocks are required in order to ensure the patient s survival The scientific literature describes cases of ventricular fibrillation in connection with high frequency surgery procedures Some pacemakers automatically modity the cardiac frequency in line with differing loads To this end they detect the atrial frequency and synchronize the ventricle frequency accordingly Here as well it is possible to provoke tachycardia by means of external interference Meanwhile devices are routinely implanted which are capable of emitting electroshocks in the case of dangerous arrhythmia These implantable cardioverter defibrillators ICDs are likewise not completely immune to external interference In potential interference situations they must be deactivated as a rule by applying a magnet and the patient closely monitored Otherwise there is a risk that the patient will receive inadequate electroshocks A 09 2008 Page 15 September 2008 As a rule modern cardiac pacemakers are implanted beneath the clavicle either Subcutaneously or in the subpectoral muscle Which approach is recommended 1 Before performing certain dental procedures it is essential to enquire in advance whether the patient is wearing a pacemaker The same due diligence applies in the case of allergies haemorrhage risks etc As a rule pacemaker users will provide such intormation of their own accord Such patients are in possession of a special

21. e viewpoint of dental practices pacemaker users should not be regarded as oroblem patients When talking to patients especially older patients about their medical histories the dentist should mention possible circulatory disorders This normally elicits information about the existence of a pacemaker or an implantable cardioverter defibrillator ICD Subject to the condition that the patient s pulse is continuously monitored either manually or automatically any properly functioning electro medical device can be applied to dental patients with the exception of electro surgery devices If these conditions are not fulfilled the patient should be referred to a hospital or clinic During electro surgical procedures the patient s heart should be continuously monitored A defibrillator should be available for immediate use After the operation has been completed the pacemaker should be tested for any possible damage If there are grounds for suspecting electromagnetic interference with the pacemaker treatment should be discontinued Advice should be sought from the patient s cardiologist or internal medicine specialist If necessary the patient should be referred to a suitably equipped hospital which caters for the special needs of pacemaker patients Before treating ICD patients the dentist should contact the hospital which performed the original implant Such prior consultation reduces the risk of complications A 09 2008 Page 17

31. ptember 2008 SCIENTIFIC OPINION German Society of Dental Oral and Craniomandibular Sciences DGZMK The dental treatment of patients with cardiac pacemakers The implantation of pacemakers has become a standard cardiological practice for the treatment of arrhythmia According to the scientific literature approx 200 000 pacemaker users were resident in Germany in 1994 It is logical that dentists are frequently confronted with such patients Articles in the popular press and scientific publications about incidents affecting pacemaker users have caused some concern This raises the following question are pacemakers affected by dental treatment procedures Background information In the early phase of their development pacemakers were fixed frequency or asynchronous devices which emitted pulses irrespective of the patient s own cardiac activity Further development led to so called demand pacemakers which were triggered only when the patient s pulse fell below a certain borderline frequency usually 70 beats per minute Cardiac activity above this borderline frequency deactivated the pacemaker A pacemaker cannot always distinguish between cardiac activity and external interference In the event of external interference stimulation may be interrupted irrespective of the patient s current cardiac status Higher frequency interference triggers a fixed rate frequency in the pacemaker In rare cases this may lead to such severe arrhythmia
