By Appaininesnt to HM Queen Elizabeth The Queen Mother Wheelchair Manufacturer IPs Bridgend Bulletin No FSNOI2 Date 4 August 2008 Urgent Field Safety Notice Invacare Reliant 100 150 200 250 Ref 2008 00 1 008 401 008 Safety Information In the interests of the safety of our customers we would like to bring to your attention that Invacare have updated the Instructions for Use having reviewed current information from our customers with end user safety in mind The operating instruction in previous issues of the user manual may have been misleading in particular the guidance given in some issues on the appropriate use of the brakes when lifting the patient was incorrect and the instructions in all issues did not make adequate reference to taking caution when moving the hoist on uneven rough surfaces and carpeted areas To improve safety while using the Reliant Lifter and to make users aware of correct use of the product it is recommended that the user attendants follow precisely the revised instruction for use as described in the product user manual Please pay particular attention to the warnings documented on page 8 and on the supplementary notification of operating instructions within the user manual Attached with this Field Safety Notice The full amended user manual will be made available via Invacare website 8 of August 2008 Please ensure that the information in this notice is made available to all
